- provide the information we need for our records if you’re a new client or have added a new pet to your household
- report changes of address and changed phone numbers
- provide an alternate contact person
- confirm you accept responsibility to pay for the services we provide your pet and most important
- authorize us to release information about your pet to veterinary specialists, emergency clinics, boarding kennels, groomers, training clubs and others with whom you want us to share your pet’s medical records
